DMARD for rheumatoid arthritis and psoriatic arthritis. Second-line after methotrexate failure or intolerance. Once-daily dosing due to long half-life of active metabolite.
How it Works
Prodrug converted to teriflunomide — inhibits DHODH, blocking de novo pyrimidine synthesis in proliferating T and B cells — reduces immune-mediated joint inflammation.

Common Side Effects
Diarrhea (most common), nausea, hair loss (usually mild), rash, elevated liver enzymes, hypertension, peripheral neuropathy (rare).

⚠️ TERATOGENIC — pregnancy absolutely contraindicated. Accelerated elimination procedure (cholestyramine) required before pregnancy. Hepatotoxicity monitoring — especially combined with methotrexate.
