What is Tamoxifen?
Tamoxifen is a triphenylethylene SERM that competitively antagonises oestrogen receptors in breast and hypothalamic/pituitary tissue. Active metabolite endoxifen (formed by CYP2D6) has ~100× higher ER affinity than tamoxifen itself and is responsible for the majority of clinical effect.
How Does Tamoxifen Work?
ER competitive antagonism → reduced oestradiol-mediated negative feedback at pituitary/hypothalamus → increased LH/FSH → HPG axis restoration. In breast tissue: ER antagonism blocks proliferative oestrogen signalling. Unlike clomiphene, tamoxifen lacks a cis-isomer agonist component.
